The Care Finder program helps older residents who may need extra assistance to navigate the aged care system. The service supports people to access My Aged Care, aged care services and other relevant community supports.

Care finders provide individual, tailored support and can assist older people with:

• Understanding available aged care services
• Arranging aged care assessments
• Finding and choosing appropriate services
• Connecting with other community and wellbeing supports
• Accessing aged care services for the first time or changing existing supports

To receive care finder support, a person must meet the following criteria:

• have no carer or support person who can help them, or
• not have a carer or support person they feel comfortable or trust to support them, and
• be eligible for government-funded aged care (check eligibility requirements using our online tool)

In addition, they should have one or more of these reasons for needing intensive support:

• have difficulty communicating because of language or literacy problems
• find it difficult to understand information and make decisions
• be reluctant to engage with aged care or government
• be in an unsafe situation if they do not receive services.

The Care Finder program is funded by the Australian Government and is available at no cost to eligible older people.
